I looked the title up in WorldCat and found that Ship has the book.
Then I clicked the "Cite/Export" button to get the citation (although it was only 95% correct).
I corrected it by comparing it to the APA Manual, and here it is:

American Psychiatric Association. (2000). Diagnostic and statistical manual of mental disorders: DSM-IV-TR. Washington, DC: American Psychiatric Association.

  2. Rico is actually correct. According to page 203 of the APA manual the correct citation is:
    American Psychiatric Association. (2000). Diagnostic and statistical manual of mental disorders: DSM-IV-TR. Washington, DC: Author.
